It was observed an infant was sleeping with a milk bottle. It was corrected during inspection.

During the investigation, it was found the operation did not ensure the incident involving the child was properly documented and communicated to the parent.

During the investigation, it was determined the operation did not ensure an incident report was completed and provided to the parent after the child sustained an injury at the operation.

During the investigation it was determined the operation did not ensure timely parent notification regarding a child's injury.

The operation's first aid kit did not have a guide to first aid. This was corrected during the inspection when a first aid guide was printed out and placed in the first aid kit.

Two of the classrooms had cleaning supplies stored in cabinets within reach of children and the child locks on the cabinets had broken. This was corrected during the inspection when the child locks were replaced so that the cabinets were locked.

A caregiver was not able to properly supervise children in care when the caregiver was taking children to the bathroom while other children were in the classroom which caused a lack of supervision and an incident occurred in the classroom without the caregivers knowledge.

Observed a trash can with dirty pull up in it without a lid. The trash was taken out and put into a trashcan with a lid.

After evaluating background check rules, it was determined that three caregivers did not have a cleared background check. All three caregivers were observed caring for children. All three caregivers were immediately removed from the operation during the inspection, bringing the facility into compliance.

During a review of the record keeping subchapter, it was determined that a fire drill had not been conducted for the previous month. The carbon monoxide and smoke detectors were not tested for the previous month and the fire extinguishers were not checked for the previous month. A fire drill was conducted during the inspection, the smoke and carbon monoxide detectors were tested and the fire extinguishers were checked, bringing the facility into compliance.

During an inspection, a caregiver was observed leaving an infant classroom out of ratio. There were seven infants (0-11 months old) left with one caregiver. A caregiver was placed in the infant classroom, bringing the facility into compliance.
