A caregiver did not wash the hands of a child after diapering.

Observed 10 caregiver files, 3 did not have proof of education. I spoke with the operation about getting the proof of education for each caregiver.

Once caregiver who administered first aid to a child in care did not have current first aid training.

An incident report form was not given to the child's parent on the day of the injury. It was sent the following week. I spoke with the director about ensuring that incident reports are given to parents right away. This was corrected when the incident report was emailed to the parent on the inspection date.

The nap mats did not have enough walking space between them to prevent other children from touching.

The kitchen and 3 classrooms had garbage cans without lids, exposing the children to the inner contents and the walls either need to be painted or covered in area where there is peeling paint or the start of repairs.

A caregiver in care left another caregiver out of ratio when the caregiver took a child to the office. The caregiver was left with 37 children and the ratio for that age group is 26 children to 1 teacher.

A caregiver in care grabbed a child aggressively by the arm to take the child to the office.

Observed dried food on the trashcans in some of the classrooms, there was also spilled liquid that has dried on the wall behind the trashccans. There was old banana's in the kitchen that had gnats over them, one of the banana's was open. The banana's were thrown away.

During an investigation it was found that a child with a food allergy did not have a completed food allergy form.

Observed a caregiver at the operation that does not have a cleared background check. This was corrected when the caregiver left the operation.

During an investigation video footage was obtained showing a caregiver grabbing a child roughly and placing the child in the bathroom.

A child was left out on the play porch without a caregiver supervising for around 2-3 minutes.

The operation manipulated a door with magnets that allowed the door to remain cracked open, which resulted in a child being able to open and close the door repeatedly and get seriously injured by the door. The operation previously had other children be injured by the same doors.

During a DFPS investigation a child was injured that required medical attention and the operation did not report the injury to licensing within 48 hours. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

An infant was observed asleep in a restricted device. This was fixed when the caregiver took the infant out of the restricted device.

During an investigation, it was found a child was able to leave the classroom without the caregivers knowing and go down the hallway unsupervised.

One classroom stated that they had 13 children but only had 12 children in care.

During an investigation it was found that a caregiver cursed in front of children at the operation.

During an investigation it was found that two allegations of prohibited punshiments were reported to the operation but was not reported to Child Care Regulation.

A caregiver disciplined their own child by hitting the child on their hand. The caregiver no longer works at the operation.

On one playground there were 2 sprinkler systems sticking out of the playground that can be a tripping hazard.

Observed the operation using a rental vehicle to transport children that does not have a child safety alarm. I spoke to the operation about putting in a variance until they are able to have their own vehicle out of the shop.

Observed 3 vehicles used for transportation. One vehicle did not have a fire extinguisher at all and the other two had expired fire extinguishers. One expired in June 2023 and the other expired in May 2023.

Observed 3 transportation vehicles, two vehicles had first aid kits that were not complete(missing thermometor, scissors and tweezers) and the third did not have a first aid kit at all.

During a walk through of the operation, there were cleaning supplies, Odo Ban and shaving cream in a cabinet that was unlocked and in the reach of the children in care. This was corrected when the items were moved to a locked closet.

The operation's handbook was missing policies including infant safe sleep, hearing and vision screenings, information about contacting Child Care Regulation and reviewing minimum standards and inspection forms, and breastfeeding.

Fire extinguishers had not been serviced since February 2023 and should be serviced annually.

Observed an infant changing mat to have a small tear. A piece of tape was used until the opration can buy a new mat.

Observed that the playground area for the toddlers was not being maintained. Observed trash and a diaper on the ground and dirty play equipment. This was corrected at inspection when the playground area was cleaned up. Observed a light switch without an outlet cover in the school age room. The was corrected at inspection when a cover was placed over the light switch.

Observed multiple classrooms that could not provide licesning with a current activity plan. The plan was also not posted outside the room where the operation usually keeps their lesson plans. This was corrected when lesson plans were added to each classroom.

Observed infant reading instructions not being updated monthly for all infants not on table food. There was one form for August and 2 forms for July and the room has 10 infant enrolled.

Observed cleaning supplies such as lysol, clorox wipes, and frebreeze in unlocked cabinets within reach of children in two toddler rooms and school age room. This was corrected at the inspection when the cleaning supplies wre moved out of the reach of children.

Based on evidence obtained during an investigation, it was found that a child was abused while in care.

Based on evidence obtained during an investigation, it was found that a form of prohibited punishment was used when a caregiver pinched a child in care resulting in a bruise.

During a walk-through of the facility, hazards were observed that included an untreated wasp nest near the playground area, ant poison near a walkway used by children during transitioning, and a loose plank near the bottom of the building that led to an area underneath the building. All of these areas were immediately corrected when addressed, bringing the facility into compliance.

After a review of children's files, a child enrolled in care did not have a food allergy emergency plan signed by a physician on file for review.

There was an electrical outlet exposed in two classrooms. Safety covers were immediately placed over the electrical outlets when addressed, bringing the facility into compliance.
