During inspection, a caregiver stated that she had 12 children but had 15.

A caregiver was aware of how many children were in the group at the playground but did not accurately count them all when leaving the playground back to class, resulting in one child being left behind.

Based on the information obtained, it was determined that a child was left unsupervised at the playground before another classroom caregiver found the child and had the child returned to their class.

10 child records were missing admission information, such as emergency contact person address, verification that parents received complete operational policies.

This standard was found deficient as part of a DFPS investigation. An incident requiring notification was not reported in June of 2025.

This standard was found deficient as part of a DFPS investigation. The operation's supervision procedures for certain age groups was determined as not sufficient enough to provide visual or auditory awareness and proximity to children during restroom use. Children were allowed to walk from their class to a restroom far enough away from the class where the caregiver was not able to maintain adequate visual or auditory supervision.

A child was unintentionally left unsupervised in the gym area for approximately 2 minutes when the rest of the class had transitioned to the outdoor playground.

An infant under 12 months old was observed swaddled while sleeping in a crib. The infant did not have a sleep exemption form on file.

Two caregivers did not have current training in infant care specific training, such as SIDS, Shaken Baby, Early Brain Development.

A bottle warner in an infant room needed to be cleaned out, as it had dried formula or water mineral build up.

Monthly playground maintenance checklists were not available for review during the inspection.

Two children were unsupervised for approximately 5 to 8 minutes. The children were found in a storage room.

Two children were found in a storage room unsupervised where cleaning supplies were stored. The cleaning supplies included hard surface and glass cleaner, toilet bowl cleaner and restroom disinfectant.

A previous incident where a child was unsupervised in a restroom due to caregivers not being aware was not reported.
