There was one room with children in care that did not have a smoke detector.

Piles of branches in outdoor active play areas. Outdoor play equipment had not been appropriately cleaned due to build up.

Pathway between indoor and outdoor active play has items blocking pathway. Outdoor area has tree branches that need to be removed.

3 children's records did not have all the required physician information, which includes the address and telephone number of the physician or hospital.

The caregiver was deficient 5 hours of annual training, which includes 3 hours of management training.

Two children's records did not have a health-care professional's statements.

The backyard had items posing a risk to health or safety, such as a broken chair with sharp parts exposed, a broken stove, used tires with water in them, as well as children's play equipment in disrepair and a sandbox filled with rain water. The back door exit leading to the backyard had tripping hazards, such as an extension cord on the floor. An indoor outlet by the Christmas tree accessible to children under 5 years old did not have an outlet cover. A craft utility knife was observed in the living room area where the children were being cared for near the dining table. A bottle of nail polish remover labeled "Keep Out of Reach of Children" was located on the dining table near the children's drinking cups.

There were no children signed in for attendance. There were 8 children present..

The sink used by children for handwashing was not clean.

Handsoap was not available in the restroom used by children. This was corrected at inspection when the caregiver put a bottle of handsoap in the restroom.

There was not a current Children's Products Certification form dated within the past 12 months.

The caregivers CPR and first aid training was expired.

Children's records did not have physician's health statements.

The smoke detector was not working in the room that children use for nap time.
