A play yard had a sheet that was jumbled up and did not fit the mattress snug. This was corrected when the sheet was removed from the mattress.

There are 4 play yards and 8 non-walking infants enrolled.

I observed a small caddy of cleaning products on the floor of the bathroom that is used by children. This was corrected when the caddy was moved out of the bathroom.

One caregiver did not have the required training on file.

There were 6 infants in the infant room with one caregiver. Specified age group is 0-11 months. Over ratio by 2. Corrected at inspection when 2 older infants were moved to the Toddler Room.

The operation did not have an updated Fire Inspection Report available for review.

The operatio did not notify parents of a safe sleep deficiency incurred on 7/19/23 within five days of the deficiency, 724/23. The deficiency was shown delinquent on 7/26/23.

Three new employees recently hired, and left alone with children still do not have at least eight hours of preservice training.

An infant was asleep in a bouncy/rocker seat for 15 minutes. This was corrected at inspection when the caregiver was prompted to move the infant to the crib.

This standard was reevaluated and continues to be out of compliance. The infant caregiver had six infants ages 4 months to 16 months and the specified age group is 12 - 17 months, which allows 5:1 ratio. This was corrected at inspection when a second caregiver entered the infant room later.

The staff CPR certification was completed on line and has not been updated to in person training.

A child was napping in a restrictive device becuase the child will not sit still. This was corrected at inspection when the caregiver released the child.

A child ran down the hall from the preschool room to the toddler room without supervision to talk to a staff. The infant caregiver left the infant room to close the door to an adjoined room.

A caregiver with four infants had an addtional three year old child in the room. Specified age group is 0-11 months which allows 4:1 ratio. This was corrected at inspection when the older child was moved to the toddler class.
