An infant class with a six-month-old was combined with a toddler class where the oldest child was three years old. This was corrected during the inspection when the infant class went back inside until the toddler class was done on the playground.

The chain link patches over gaps in the fence were starting to come loose, resulting in loose wires protuding from the fence.

There was a mop bucket with water in it stored in a bathroom that was accessible to children. This was corrected during the inspection when mop bucket was emptied.

The director stated that the school-age children were using the field next to the operation, which had not previously been used or measured. This was corrected during the inspection when the director stated that they would no longer use the field.

During the investigation, there was sufficient evidence found to determine that caregivers were not adequately aware of children's ongoing activities as a child in care was able to capture a mouse on the playground and was bitten.

There was a hole in the fence of the playground that was large enough for a child to crawl through.

Out of six children's records reviewed, three were missing physician and emergency medical facility information, two were missing a parent's signature for consent to emergency medical treatment, one was missing hearing and vision screening results, and one was missing a parent's acknowledgment that they had received the operational policies.
