During a DFPS investigation it was made known that the operation failed to report situations to CCR regarding an injured child at the operation that received medical attention.

The operation was found to have expired medication on file for five children

It was found that the operation did not notify parents when two children were participating in appropriate acts.

It was found that a caregiver did not intervene while two children were participating in appropriate acts.

A caregiver pulled a child up from the floor by his arm.

A caregiver yells at children as a form of discipline.

The caregiver made a threat to whoop a child and hit the child in the back. The same caregiver also threatened the class after hitting the child.

This standard was reviewed in part of a DFPS investigation and found to be deficient. A caregiver did not properly supervise their group of children.

A child was not supervised after arrival when the operation did not allow the child's entry and the child walked away unattended. Staff were not aware of the child's location.

Operation staff did not use good judgment when they did not allow an enrolled child to attend after school care upon arrival.

Infants were not strapped securely while sitting at the tabletops. This was corrected at inspection when the children's straps were fastened while they sat at the tabletop.

Children under 2 years old were allowed to watch television. This was corrected at inspection when the televisions were turned off. There is also a plan to use tabletop centers for children under 2 who arrive at the center early.

This report was reviewed in part of a report and found to be deficient. A caregiver physically moved children to a time out chair by lifting them roughly.

A child was left on a vehicle unattended

A child was neglected while in care.

The driver did not account for all children before leaving the bus unattended.

The driver did not demonstrate competency in not following the operation's policies for transportation. The driver did not inspect the inside of the vehicle before turning off the safety alarm.

This standard was evaluated due to a DFPS investigation and was found to not be compliant. A caregiver did not have visual and or auditory awareness for approximately 1 hour when child was on a vehicle outside alone.
