A child was misising the health statement in the file.

During the inspection, a child was able to leave the toddler class and open the front door without supervision.

During the follow up completed one staff member was present in the building without a valid background check.

Based on the investigation completed it was found that a staff member was using a personal cell phone while supervising the children at naptime.

Based on the investigation completed it was found that a caregiver was allowed to work at the daycare with an ineligible background check.

One out of six child records reviewed did not have the medical authorization on file.

It was found that the van drivers transportation training expired on 08/28/2024 and was completed 08/28/2023.

One child did not wash their hands after using the restroom. One classroom did not wash their hands after outdoor activities.

The operation did not have documentation to show they had a current health inspection but stated the health inspector had come to conduct the inspection. The last documented inspection was 6/22/22.

The operation does not have a current fire inspection. The last fire inspection was conducted on 8/17/22.

There is evidence to show that a caregiver has hit, thumped and flicked children as a form of discipline.

Based on the information obtained during the DFPS investigation there is evidence to show that screentime is used with infants as they watch the show Cocomelon.

The first aid kit was missing the tweezers.

The Children's Product Certification form has not been completed since June 2020.

The first aid kit had expired alcohol pads.
