A caregiver didn't demonstrate positive discipline methods when they swatted at a child with their hand to get the child to comply and aggressively held and guided another child's arms to use them to clean up.

There was observed dirt and food crumbs on the floors throughout the operation. This was corrected by the floors being swept and mopped during the inspection.

The playground was measured to have less than 6 inches of loose-fill.

The two restrooms being utilized by children in care were not equipped with hand soap or disposable paper towels. This was corrected by hand soap and paper towels being placed in the restrooms for the children's use.

The elevated play structure on the playground was observed to have peeling sharp edges on the steps. There was also observed an active ant bed inside the playground area.

During the inspection it was observed that the diaper changing mat had tears, exposing the foam.

Caregivers did not provide the correct ages of the children they were caring for when asked.

I observed the playground to have less than 6 inches of loose-fill.

I observed a 5 month old being bottle feed while laying in a crib.

I observed a infant sleeping in a restrictive device and the caregiver verbally confirmed the child had been asleep for 5 minutes.

I observed an infant crying without being held and comforted. This was corrected when CCR staff asked staff to pick the child up.

Toilets, sinks, doors and floors are not being kept clean. The toilets have rings, a toilet brush was observed on the floor in the restroom during the walkthrough. Doors and floors have stains and dirt throughout the daycare.

During the inspection it was observed that a new hire was in the classroom observing with a pending background check.

During the inspection it was observed that the diaper changing mat had tears, exposing the foam. Note: This was corrected when a new one was ordered.

The first aid kit contained expired alcohol wipes; which expired in 2017. Note when brought to the director's attention expired wipes were removed and replaced with wipes that were not expired.

During the follow up it was observed that the maintenance items (Such as the toilet and floor boards and outside entry way) were pending repairs. Note: A work order has been made to get everything repaired this week.

During the walk through it was observed that one restroom had toilet paper clogged and toilet paper everywhere on the floor. The floors boards in the restroom and toddler room are broken and open. It was also observed that the outside entry way to the playground has dirt and water making hard for children to cross inside.

A child in the infant did not get their hands washed after having the diaper changed. This was corrected at inspection.

During the follow up inspection it was found that one caregiver was counted in ratio with a pending background check.

In one room the nap cot chart was missing. This was corrected at inspection.

One classroom did not have the roaster showing the name and ages of children.

One employee in the infant room did not wash hands after changing diaper. This was corrected at inspection.

One infant observed sleeping in the crib with pacifier cord attached. Note: This was corrected at inspection when it was removed.

One child was observed sleeping in a restrictive device. Child was had been sleeping for 10 minutes. Note: This was corrected when the child was moved to the crib. Pictures were taken.

It was found that caregiver records are not available for review at the operation.

An infant was observed sleeping in the crib with the bib on. Note: This was corrected at inspection when the bib was removed.

During the inspection it was observed that the mulch (surfacing) is not to proper depth. It was observed that there is more dirt than mulch. Pictures were taken.

A video was observed provided by Director, 2 children were seen unsupervised. There were a total of 3 caregivers in the building at the time of the incident. All 3 caregivers were observed doing other duties.
