CCR staff observed the television on and asked operation staff did the lesson plan for the day go along with the show the children were watching. The caregiver stated no and turned the television off.

During the walk through CCR staff observed two toilets clogged and operation staff was not aware of how long the toilets have been clogged.

When CCR staff arrived to the operation the 3-4 year old classroom was out of ratio. One caregiver was caring for 19 children SAG 4 years old.

During the walk through a caregiver provided the incorrect number of children they were responsible for.

During the walk through the toddler classroom did not have an evacuation and relocation diagram posted.

During the walk through CCR staff observed two infant in highchairs without safety staps fastened. This was corrected when the caregiver fastened the straps.

During the walkthrough a caregiver provided the incorrect number of children they were responsible for twice when asked. When CCR staff counted the children neither of number of children provided by the caregiver was correct.

During the walk through a caregiver provided incorrect ages of children in their class.

During the walkthrough there was two classrooms with children under two with the television on.

During the investigation incitation inspection operation staff confirmed a staff member was allowed to work at the operation without a cleared background check. The background check was submitted but their fingerprints had not been completed.

During the walkthrough one caregiver was caring for 19 children SAG 2 years old.

Based on the information obtained throughout the investigation, if was found that the director did not make sure the operation was operating in compliance with minimum standards daily as the daycare has been found to be out of ratio and caregivers have been allowed to work without background checks.

An infant was observed in a bouncer without safety straps fastened. This was corrected when the child was removed from the bouncer.

The daily reports did not include a summary of activities during the inspection.

The daily report did not include times of diaper changes during the inspection.

Cribs were not labeled for the children present in the classroom. This was corrected when the operation made labels and posted them on the wall above the cribs.

Bottles/cups were observed in the infant class unlabeled. This was corrected when the caregiver labeled the bottles.

The daily report did not include the infants mood during the inspection;.

The daily report did not include the nap times during the inspection.

Infant feeding instructions were not available for review during the inspection.

There was one caregiver present at the operation with an inactive background check. This was corrected when the caregiver left the operation and the director ran her background check.

Two vans used to transport children did not have an electronic child safety alarm.

One of two vans used to transport youth in care did not contain a fire extinguisher or first aid kit. The director corrected this by placing a first aid kit and fire extinguisher in the van.

One out of four employee files reviewed did not contain documentation CPR/First Aid training had been completed.

One sink was observed to be utilized for both handwashing and preparation of bottles in the infant classroom. The caregiver corrected this by removing bottles away from the handwashing sink and diapering area.

An infant was observed asleep in a swing. The caregiver corrected this by removing the child from the swing and placed the child in a crib.

There were soiled diapers observed to be loose in an open trash can in one of the classrooms.

In the two-year-old to three-year-old classroom, cough medicine was found on the desk and accessible to children. Note: This was corrected when staff removed the medication and placed out of reach from the children.

During the investigation, three children disclosed that a "whoppin" was used as a discipline method at the operation.

During the walk through, two children aged four and age five in the toddler classroom were sitting in highchairs from 9:39am to 10:06am. In addition, a four-year-old was observed in a restrictive device for time out.

During the follow up inspection, one child did not have immunizations on file and one child did not have updated immunizations.

The operation did not have an up to date fire inspection. The most recent fire inspection was completed on 9/13/2023.

During the inspection I observed 12 children to be present in the infant room. Based on the specified age group, the room exceeded the maximum group size of 10. NOTE: During the insepction, two children were moved to another classroom to maintain ratio.

I evaluated 10 childrens records and the following was observed: - One child did not have an emergency care authorization - One child did not have a phone number or address for the emergency contact. - One child did not have record of immunizations - One child did not have an up to date record of immunizations

One caregiver with a pending background check was observed to be present in the classroom and caring for children. NOTE: This was corrected at inspection when the caregiver left the operation.

In the toddler class, cough medicine was found in an accessible small storage bin drawer. The cough medicine was in the 3rd drawer from the bottom which is reachable for the toddler children. This was corrected when the director removed the medicine from the drawer and placing it in the office.

One infant was observed sleeping in the crib with pacifier cord attached.

One caregiver present in the infant room did not have valid background checks. The background check is inactive. Note: Caregvier was asked to leave.

One infant was observed asleep in a restrictive device (Bouncer).

One toddler classroom did not have the nap chart showing which cot belonged which child. Note: This was corrected when the nap chart was made.

Eight out of ten caregivers files reviewed is missing the educations qualifications on file.

One out of ten child records reviewed is missing the health statement on file.

The fire drills for september was not documented.

One out of ten child records reviewed is missing the immunization record on file.

During the walkthrough one toddler room did not have the fire exit diagram posted. Note: This was corrected at iinspection when it was posted.

Three infants were observed sleeping in the crib with blankets, bottles and bibs. 5 month old had a bib on and bottle inside the crib. 10 month old had a blanket inside the crib 10 month old had a bottle inside the crib.

It was observed that in the toddler room a cot being used by a toddler was blocking the exit.

A classroom with 19 children in care ages 3-4yrs old was observed napping without an adult supervision.

5 month Infant was observed sleeping in crib with a pacifier attachment and bib around her neck.

During investigation Director was not aware of child with a medical statement declaring a child food allergy.

During investigation a list of children in care with food allergies was not observed.

During investigation food allergy plan for child enrolled with a food allergy could not be observed.

During follow up 3 of the 6 caregivers folder did not have clock hour trainings.

2 infants 12months and younger were observed sleeping with blankets, wipes and pampers in crib.

3 caregivers were observed caring for children with an active background check.

Current annual fire inspection could not be observed. Fire inspection observed dated 7/25/2019.

There were 2 caregivers observed with 9 infants in care age ranges from 4-12months.

2 infants were observed in high chairs with safety straps unstrapped.

Mulch was not observed on the outside play area.

A caregiver was observed in the Infant classroom without an active background. Corrected at inspection: caregiver was asked to leave.
