15 children enrolled in the school did not have updated documentation of the required immunizations required by the Texas Department of State Health Services.

One staff file was missing the notarized affidavit form.

After reviewing 10 child files, one child was missing a statement of health from a health-care professional.

Observed 3 classrooms with unlocked or broken closet doors which stored cleaning supplies such as windex in the reach of the children. This was corrected when the doors were locked and the chemicals were placed out of the reach of the children for the door that was broken and could not be locked.

Observed 8 staff files and 4 were missing training hours or did not have any of the required 24 hours of anual trainings.

Observed an infant sleeping in a swing, this was corrected when the caregiver removed the infant from the swing.

Observed a 5 month old infant sleeping in a crib with a burp cloth over the infants face. This was corrected when the cloth was removed from the infants crib.

A child was left unattended on the playground for roughly 28 minutes.

The operational policies were missing the following items 18) Promotion of indoor and outdoor physical activity that is consistent with Subchapter F of this chapter (relating to Developmental Activities and Activity Plan); your policies must include: (A) The benefits of physical activity and outdoor play; (B) The duration of physical activity at your operation, both indoor and outdoor; (C) The type of physical activity (structured and unstructured) that children may engage in at your operation; (D) Each setting in which your physical activity program will take place; (E) The recommended clothing and footwear that will allow a child to participate freely and safely in physical activities; and (F) A plan to ensure physical activity occurs on days when extreme weather conditions prohibit or limit outdoor play

The program is not following all the meal and snack patterns established by the established by the USDA Child and Adult Care Food Program.

Two child files were missing current immunization records.

A caregiver was present in a classroom who did not have a current eligible background check. The caregiver left during the inspection.

Observed two caregivers without a cleared background check caring for children. This was corrected when the caregivers left the operation.

Out of 10 staff files reviewed, 3 caregivers had taken first aid and cpr training online and did not use the minikin. The standard was reveiwed with the directors and they have CPR scheduled for all staff on 4/15/2022.

The toddler play area has a play structure was able to get hot enough to burn a toddler's hand. This was corrected at inspection when the operation removed the play structure.
