During today's inspection employee was observed as person in charge and being unsupervised with children that is in violation with conditions of employee's pervional background.

4 children's admission forms were reviewed during today's follow up inspection. 3 of the admission forms still need to include someone other than mom and dad as the emergency contact unless noted that mom and dad are the only emergency contacts. 1 form still needs list address information for emergency contact.
