Fire inspection records were reviewed during the unannounced inspection. The most recent inspection report was from 11/7/23.

It has been determined that a caregiver did not demonstrate competency and good judgment by not following the bottle identification procedures when feeding a breastmilk bottle to an infant. Bottles are labeled, color-coded for each child, stored in labeled trays for each child, and breastmilk bottles are placed in baggies in the refrigerator for further distinction. Failure to follow procedures resulted in a child being fed the wrong bottle that contained breastmilk, an exposure to a body fluid, placing the child at risk.

1 infant needs feeding instructions to be signed and dated by parent.

The operation did not have a current annual fire Inspection.

Two children with diagnosed allergies did not have plans signed by the Doctor and the parent.

10 child records were reviewed. 1 child needs emergency contact information for admission form and 1child needs addresses for emergency contacts.

Director needs to complete annual transportation training.

Infant feeding instructions were reviewed. 1 infant needs feeding instructions.

10 child files were reviewed. I child needs an updated immunization record.

Based on the information received during the investigation it was found that the caregiver was cussing at the children.

Based on the information received during the investigation it was found that the caregiver pinched a child.

Based on the information received during the investigation the director didn't disclose to the parent that there were concerns that the caregiver had spanked the child.
