It was observed during today's walk through of the operation that there were two holes in a wall in the infant inchworm classroom and 1 in the ladybug classroom.

This standard was reviewed in this investigation. Based on the information provided, staff were on their cell phones while supervising children during naptime. Staff wasn't performing school duties on their cell phones.

This standard was reviewed for this investigation. Based on the information provided, a staff member applied pressure on a child lying on a nap cot to prevent a child from moving during naptime.

Based on the information gathered during the investigation, it has been determined the operation failed to report a child was left unattended to Child Care Regulations as soon as possible but no later than two days after the incident took place.
