Operation has not maintained a record of attendance for each child in care. Sign in and out logs were sporadically completed. This was corrected at inspection by logging today's attendance.

There was a person present, living in the home with a pending background check that has access to the children in care. This was corrected at inspection by having the person leave the home.

Operation does not have documented safety documentation. This was corrected at inspection by completing a fire drill and documenting safety equipment checks.

The caregiver was seen changing diapers of two children and did not wash their hands.

The fire drills reviewed at the home exceed the three minutes. The max that it took the operation to exit the home was 10 minutes in some drills.

The caregiver did not have the required 30 hours of training for her current year. There were no HASA specific trainings completed.

Of the five children's records evaluated, one child did not have a health statement on file.

The primary caregiver only obtained 11 of the required 30 clock hours of training for the previous training year 5/2020 - 5/2021. The primary caregiver will need to receive 19 clock hours of make-up training. The make-up hours will not be counted towards the current training year. The primary caregiver will need to obtain the following topics of trainings: *SIDS/SB/EBD *Emergency preparedness *Preventing and controlling the spread of communicable diseases *Administering medication *Preventing and responding to emergencies due to food or an allergic reaction *Understanding building and physical premises safety *Handling, storing, and disposing of hazardous materials *At least three of the annual training hours in management techniques, leadership, or staff supervision
