It was observed that trash can in the two year old room that was full of trash did not have a lid cover. The trash in was by the handwashing sink that accessible to children.

Children haven't been outside for almost two weeks due to ants, bees and wasps.

Several small ants mount observed in the playground.

It was observed that the diaper container in the Two year old room did not have a lid cover.

5/7/24: The owner/director did not follow the minimum standard rules by having more children then capacity allowed. 4/19/14: The owner/director did not follow the minimum standard rules by not notify Licensing when she relocates children to her residence. Children and caregivers were at her residence for two weeks when the operation is closed due to not paying the rent.

Proof of current liability insurance was not available for review at inspection. Liability insurance expired on 3/04/2024.

It was observed a caregiver with provisional eligible with conditions alone with children. She was observed in a four year old class by herself. She also observed in the two year old class by herself when the other caregiver takes children to the restroom. She's also by herself when she took children to the restroom.

CPR and First Aid needed for a caregiver who is in charge of a group of children.

It was observed that there was no light in the bathroom. The light went out since April 19, 2024.

Upon arrival I observed the two year old class has 13 children with one caregiver. This was fixed during inspection by adding another caregiver to the room. I also observed a caregiver left the two year old room to go prepare lunch. Which left one caregiver with 15 children in the room. They're 4 children over child/staff ratio.

It was observed that there are 23 children in care during inspection. They are 6 children over the capacity.

The carseat the infant was sleeping in was covered by a blanket.

An infant was asleep in the car seat. He was removed when the caregiver was informed that he could not sleep in there.

An infant was in care at the time of the inspection. The facility is not licensed to care for infants.

The facility did not have a current fire inspection.

An infant was in care with no records or documentation.

The facility did not have the most recent inspection form posted.

Policies read during the inspection did not address several required standards.
