While conducting a walk though of the operation, there was a 4 month old sleeping in a restrictive device and swaddled in a blanket.

This standard was reviewed during an investigation and found deficient. Parents are not being told about less serious injuries at the end of the day.

This standard was reviewed in relation to the DFPS investigation and found to be deficient. Acceptable behavior was not demonstrated when a caregiver who no longer works for the operation was seen flicking a child as a form of discipline.

This standard was reviewed in relation to the allegations and found to be deficient. A child was found alone and unharmed in a classroom by a collateral parent.

During the inspection, it was observed by licensing a caregiver present at the operation without a current background check supervising a group of 26 children. This was corrected at the inspection when the caregiver left while licensing was still present.

Based on the allegation and information provided during the investigation, the standard was found to be deficient. A caregiver that is no longer employed at the center embarrassed and pulled a child who was having a wardrobe malfunction.

Based on the information obtain during interviews and observation, two caregivers are left supervising children alone which is against their current background check conditions.
