Loose-fill was not properly maintained in an area with a play structure taller than 5 feet. The loose-fill measured between 2-4 inches when it was required to be a minimum of 9 inches in depth. The director agreed that children will not be allowed to play on the structure until the operation is able to bring in more loose-fill to reach the depth as required by minimum standards for child safety.
