The laundry room door was open and cleaning chemicals were observed to be accessible to children. This was corrected at inspection where the director was able to close the door, and it was able to lock.

The caregiver was unable to provide an approximate time of how long the children were in the bathroom together unattended. Prior to the incident, another child walked into the bathroom on the same child.

During the investigation the operation failed to report the child's injury which required medical attention.

During the review of the video provided the staff did not intervene enough to provide the interaction of the other children from being hit or bit.

During the inspection it was found that the operation had not documented that they have completed a fire drill since June 2024.

During the inspection it was found that one of the transportation vehicles was missing it's first aid kit, and some of the other first aid kits in the other vehicles were missing items. This was corrected at inspection when all of the first aid kits were made complete and one was added to the vehicle that did not have one.

During the inspection it was found that 3/5 of the first aid kits reviewed contained expired supplies. This was corrected at inspection when the expired items were removed from the first aid kit.

There were two shelves in the bathrooms of the pre k rooms that were not secured to the floor or wall, creating a tipping hazard.

A caregiver did not properly supervise her classroom when exiting the playground. A name to face count was not conducted; leaving a child unattended on the playground.

Two caregivers did not demonstrate good judgement. Both caregivers witnessed a child unattended outside and did not report this to the director.

One employee at the operation was found reason to believe for Neglect by Department of Family and Protective Services.

Follow Up: The parent still had not signed the incident report.

Based on information obtained during the investigation, there is sufficient evidence to support a citation that parents are not allowed inside the operation.

Based on external documentation and interviews, incident report was not signed by the parents.

Based on the information obtained during the investigation, parents did not receive a written notice of a lice outbreak.
