At the time of the inspection a fire inspection had not been conducted within the 12 previous months.

The caregiver is not washing hands after changing diapers.

Required Affidavits are missing from 3 of the 6 employee records evaluated.

The Pre-K play area tarp surfacing is exposed and presents a tripping hazard for the children.

Annual trainings not completed for 1 of the 6 employee records evaluated.

1 person CPR training was missing for an employee.

The children's hands are not being washed in between diaper changes before interacting with the other children.

Based on information obtained in an investigation the operation did not have a policy in place to record and verify the identify of an individual that picked up a child in care.

It was found that a parent updated the child's enrollment record by requesting a person be added as a designated pick up person, but the enrollment record was not physically updated and a signature of the parent was not obtained to verify the update.

During the inspection it was found that 1 child did not have emergency medical authorization information.

During the inspection it was found that more than 12 months had passed since the last Child Product Safety document was certified and signed by the director.

At the time of inspection, the director did not have required transportation training available for review.

At the time of inspection, 2 caregivers did not have current CPR/First Aid certification available for review.

At the time of inspection, current annual training for several staff was not available for review.

At the time of inspection the outdoor activity areas were observed with litter and overgrown grass/weeds. There was also a border along the fence with sections of it coming out of the ground posing a tripping risk. There was also a broken vent observed outside of the building in the Toddler area accessible to children and a large vine growing outside of the door with spikes which was also accessible to children. Additionally in the toddler outdoor area, there was an open sprinkler control panel with exposed wires accessible to children.
