It was observed that during nap time, one caregiver was seated between two classrooms of napping children (one classroom with 18 children ages 3-5 and another classroom with 15 children ages 4-7 years old). Another caregiver arrived during the inspection correcting the nap time ratio.

During an investigation, it was determined that a child was released to someone without checking their identification or verifying they were on the child's authorized pick-up list.

During the inspection, ten infants were observed in one classroom with one caregiver. Also, twenty-four two-year-old children were observed in one classroom with one caregiver.

The designated person in charge did not have access to the operation's records. The person in charge stated children's records and personnel records are kept in a locked cabinent and she does not have a key to unlock it.

A towel was observed in a crib with an infant younger than 12 months. The caregiver removed the towel from the crib.

A 5 month old infant was observed drinking a bottle that was propped with a towel. The towel was removed and the caregiver carried the infant to feed them.
