The bus binder did not have children's medical treatment authorization forms. This was corrected during the inspection when copies of medical treatment authorization forms for the children who ride the bus were created and added to the bus binder.

The operation had two Epi-Pens for a child, and one of them was expired. This was corrected when the expired Epi-Pen was sent home with the child's parent.

The parent handbook was missing policies for daily health checks and an infant safe sleep policy.

A child who had a diagnosed food allergy did not have a completed allergy plan on file.

During the investigation, there was sufficient evidence found to determine that a caregiver did not have the physical ability to meet the needs of the group they were caring for.

Out of the six children's records reviewed, one was missing hearing and vision screening results and another was missing school information, permission to ride the bus from school, and a statement from the parent that hearing and vision screening results were on file with the school.

There was a 16-month-old in a classroom where the oldest child was 3 years old. This was corrected during the inspection when the 16-month-old was moved to the infant classroom.

Two classrooms did not have an evacuation diagram posted. This was corrected during the inspection when copies of the evacuation diagram were printed and posted in the rooms.

Out of six staff files reviewed, one was missing a notarized affidavit, one was missing proof of education, and three were missing a statement that they attended orienation with an overview of abuse and neglect policies. Two of the caregivers signed orientation statements during the inspection.

An infant classroom was observed to be used as a passgeway for older children, parents and staff during the inspection.

The borders around the loose fill on the school age playground are warpped and falling a part creating tripping hazards. There is a play structure on the toddler playground that is not put together per the manufacturer instructions- the missing pieces were set aside on the playground and the children were climbing on them. The director moved the loose pieces. There are tires on the school age and toddler playgrounds as well as at the entrance of the school age playground. Photos were taken.

There was a bucket of inscect repellant that read 'keep out of reach of children' in the 4-year-old classroom that was accessible to children.

The rocking equipment on the playgrounds were not anchored properly on any of the playgrounds. Photos were taken.

A child's parent has instructed the operation to administer 2 mL of a medication that requires a doctors note for the age of the child.

The operation added rocking equipment on all playgrounds in May and shifted the equipment which has caused none of the equipment to maintain proper use zones.
