The parent handbook was missing an infant safe sleep policy, procedures for parents to communicate with the director, information on the abuse and neglect hotline and how parents can access minimum standards online, procedures for nursing mothers to nurse their children at the center, and information about the benefits of physical play and how children will do active play when the weather prohibits them from going outside. This was corrected during the inspection when this information was added to the handbook.

A caregiver was heard yelling at children during the inspection. This was corrected during the inspection when the caregiver was reminded that yelling is not an acceptable form of discipline.

The first aid kit in the transportation bus was missing a guide to first aid, tweezers, and antiseptic wipes.

Out of ten children's files reviewed, three were missing hearing and vision screening results and two needed updated immunization records.

During the DFPS investigation, there was sufficient evidence to show that a caregiver stepped into the bathroom to complete diaper changes, leaving the children in the classroom unsupervised.

A caregiver who had conditions on her background check and couldn't be left alone with children was left alone with children during naptime. This was corrected during the inspection when another caregiver returned to the classroom.

There were two caregivers in classrooms who did not have a cleared background check yet. This was corrected during the inspection when the caregivers left the operation.

One classroom of 5 infants, ages 10 - 13 months, did not have their hands washed upon returning inside from outdoor play.

It was observed that the fence on the playground was not secured properly. Children were observed lifting the bottom of the chain link fence posing a safety risk.

One infant was observed to be swaddled in a bare crib during the inspection. The swaddle was removed.
