Based on information gathered during the DFPS investigation evidence supports that a caregiver did not cleanse an infant with an individual cloth or disposable wipe after a soiled diaper was removed during a diaper change:

loose staples and sand were located under and around the diaper changing mat in the two year old room and the 12 to 18 month room. This was corrected at inspection and the mats and tables were cleaned.

In the infant room one Infant Care Instruction sheet for was 4 days past due to be updated. The form was last signed on 7-23-25 and is required to be updated every 30 days.

A Review of the Employees Who Left Employment in 2024 data from January 22, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

One bottle was not labeled. This was corrected at inspection when a label was placed.

The Director engaged in conducting a recorded forensic interview of a 3-year old child in which the Director is not properly trained to administer.
