Based on DFPS investigation conducted, a 2-year-old child was left unsupervised for 10 minutes on the playground.

Based on the DFPS investigation conducted, staff did not communicate with each other about combining classrooms and how many children staff would be responsible for.

Based on DFPS investigation conducted, it was determined there was one teacher to fourteen 2-year-olds in a classroom. The maximum number of children one caregiver can supervise is 11, so the room was over ratio by 3 children.

Based on DFPS investigation conducted, caregiver was observed on cell phone while supervising children on the playground.

At time of inpsection, staff who administered medication, did not document their full name on medication log.

At time of inspection, one caregiver (expired 10/2024) and director (expired 2/2025) had expired Pediatric First Aid Cards. Noted: First Aid training class is scheduled for today at noon.

At time of inspection, one out of the ten files reviewed. One child was missing emergency contact information on admission form.

At time of inspection, one out of the ten files reviewed. One child was missing authorized child pick up information on admission form.

At time of inspection, one out of ten child files reviewed. One child was missing current immunization record in their file.

At time of inspection, two out of the ten files reviewed. One staff member had 13 hours of annual training and the second staff member had 18 hours of annual training for the 2024-2025 year.

At time of inspection, one caregiver (expired 10/2024) and director (expired 2/2025) had expired Pediatric CPR Cards. Noted: CPR training class is scheduled for today at noon.

At the time of inspection, nine children's medication had expired.

At time of inpsection, two out of the ten children files reviewed. Two children were missing physician and or emergency facility information on admission form.

One child's file did not have a copy of the immunzation recrod.

A review of the Employees Who Left Employment in 2024 Survey data from January 17, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

In the infant classroom one caregiver was caring for 5 infants ages, 3 months, 3 months, 5 months, 5 months and 6 months. The classrom was over ratio by one infant. The infant was moved to the other infant classroom bring the classroom back into ratio.

Based on the information during the investigation, there is sufficient evidence to support the caregiver using their personal device while supervising children.

Medications in the center were labeled with the children's names, but not the date brought into the center.

Two medications were for children no longer enrolled in the operation. This was corrected during the inspection when the medications were discarded.

The toddlers in the Toddler B room were watching a sing and dance music show on the television. The children in this room are 17 to 19 months old.

Based on the Preponderance of evidence found during the DFPS investigation, a center employee younger than 18 years old was alone in a classroom with a group of children.

Based on the preponderance of information obtained during the investigation a person in charge of the operation was multi-tasking while caring for children. The caregiver left the room multiple times, and was handling other duties, which took their attention away from maintaining supervision. As a result, a child was able to exit the doors of the operation.

Based on the evidence obtained during the DFPS investigation, there is sufficient evidence to support the allegation of neglectful supervision.

Based on the preponderance of information obtained during the investigation a caregiver failed to adequately intervene before a child was able to exit the building.

The director files did not contain the following training: Handling, Storing, and Disposing of Hazardous Materials, Building and Premises Safety, and Preventing and Responding to Emergencies due to Food Allergies.

Director and four staff files did not contain an updated transportation training,

Cots located in the preschool classroom were observed lacking walking space in between them.

One trashcan was observed uncovered in the preschool classroom.

Four of ten staff files did not have annual training in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Building and Premises Safety, and Preventing and Responding to Emergencies due to Food Allergies.

A 3 month old and 4 month old infant was observed asleep in swings. SN: Pictures were taken.

Documentation of a current fire inspection could not be provided.

Changing mats were observed to be torn in the infant classroom.

One caregiver was observed with five infants putting the classroom over ratio by one infant.

The pre-k classroom was observed watching elmo during lunch time.

A 7 month old infant was observed sleeping in a swing with a blanket. SN: Pictures were taken.

The last fire drill was documented 10/1/21.
