It was discovered during the investigation that a caregiver did not demonstrate self-control and good judgement when they threw a chair in a rough manner down that a child was carrying and grabbed a child by their arm due to them not apologizing to another child for their behavior.

This standard was reviewed as part of an investigation and found non-compliant based on information obtained. It was discovered that a caregiver was not following feeding instructions for a child when they feed the child cereal and baby food prior to parent giving permission.

During the investigation it was determined that the center failed to report an incident that a staff had allegedly inappropriately picked a child up in a timely manner.

During following inspection it was observed that several nap mats being used by children were ripped and in the toddler classroom a baseboard was missing exposing the wall that was peeling.

Trash was observed on the playgrounds along with broken toys, trash was on the floors and sinks in school age classroom, the floor and the toilet in boy's school age classroom room was dirty and the girl's school age bathroom was out of order and lastly a rip was observed in the diaper changing mat in the infant II classroom.

During the investigation photos were taken and the infant/toddler playground a gate leading to the parking lot had a gap that a child could fit through. On the pre-k playground the bottom of the fencing was loose that a child could get under the fence and lastly a child is able to fit between fencing and fence post.
