Kids Heaven Daycare and Learning Center

The infant and toddler rooms have paint peeling from the walls. In the hallway the air intake vents are visibly dirty with dust and grime.

Two bottles of an unknown liquid were observed in the kitchen on top of the fridge. Note: The director said they were just bottles of water and wrote water on the bottles.

The child safety alarm is not being maintained to the manufactures recommendation. The alarm has to be unplugged every night to ensure it does not drain the battery. It also causes the radio to turn on when the keys are not in the ignition. The alarm did not work properly when the director returned back from the bus run.

The operation did not have a current fire inspection available for review.

Two children enrolled in care did not have current immunization on file and available for review.

The following information was missing from the children's admission information: - 1 child did not have the required physician health statement on file and available for review. - 1 child did not have the required authorization for medical emergency attention information - 2 children did not have the required public-school information.

A child with a diagnosed food allergy did not have a sign and dated food allergy emergency pla by the parent or the physician.

Caregivers/staff was missing the required following personnel information: 1 out of 4 staff did not have proof of education. 1 out of 4 caregiver did not have a notarized affidavit

Transportation training was not observed in the director's file.

Two children's files out of 8 reviewed are missing the health statement for the child.

Copies of the CPR and first aid were not in the file for the director and two caregivers.

Two gallons of milk were observed in the fridge past the expiration date of 2/10/25. Note: These two gallons were dumped out during my inspection.

The youngest child, 13 months old, is more than 18 months younger than the oldest child in the room at 4 years old. Note: One child went home leaving the operation to have 12 children in care.

The fire inspection has not been completed within the 12 months of the previous inspection and not before the current application.

The keeping children safe flyer is not posted.

A list of current employees in not posted in a predominant place.

The laminate on the tiles in the school age room are pealing up. The infant room also has cracked tiles near the door.

Wood boards on the climbing structure are beginning to rot. The drinking fountain on the playground is falling apart and exposing electrical wires and the motor. The loose fill area has grass growing. The loose fill is packed down causing it to not have the shock absorption.

The required flyer was not observed posted.

The CPCS was not observed posted.
