The operation did not follow the conditions for a caregiver with a variance. The caregiver should not have been left alone with children.

A child under the age of 12 years old was observed sitting in the front seat. Child stated she sit in the front when a lot of children are on the van. Corrected: When front office administrator had the child to seat in the back of the van.

Five caregivers that were hired after September 2023 did not have a Pre-Employment Affidavit on file.

Two caregivers did not have information on their education and two caregivers did not have the required education.

Four children did not have someone besides a parent as an emergency contact.

One drive, observed driving children to school did not have an driver's license.

An employee had a pending background and had access to children when driving them to school and supervising children in the classroom. Corrected: When the caregiver was sent home.

Three caregivers did not have an Affidavit on file.

Three children did not have immunizations records on file at the operation.

Four children did not have a booster seat to seat in, the seat belt did not go over the chest or lap area correctly. Corrected: When the director provided booster seats for the four children.

A two-year-old and four-year-old children were seen sleeping in restricted devices. It was corrected at the inspection time when children were removed from the devices and placed in cots during naptime.

The operation has not provided parents with a copy of the parent's rights form 2987 as required.

Two out of eight children did not have immunization records available to review during the inspection time.

Two children with special care needs did not have a statement of the child's special care needs in their records.

One employee did not have availble to review the training hours during the inspection.

2 buckets filled with water was observed to be accessible to children inside the kitchen. There was also a children?s toy box observed to be filled with rainwater on the playground. *This was corrected by the water being emptied and the toy box being brought inside, and the kitchen door being closed.

The kitchen door was observed to be open with chemicals on the ground within children?s reach. *This was corrected by the chemicals being removed from the ground and the kitchen door being closed.

5 out of 7 employee records reviewed did not have an affidavit of employment on file.
