Caregiver left child alone in the front room while they went to make breakfast and to answer the front door.

Children were observed having more than a hour of screen time that had nothing to do with plan activities.

One of the vans did not have a working child safety alarm.

A seven month old infant was sleeping in a restrictive device. Corrected: When caregiver removed him and placed the child in a crib.

Children under the age of two years old were sitting in front of the television.

A mix group of 14 children in care were placed with infants. The oldest child in care was 18 months older than the youngest child in care.

The operation did not notify Child Care Regulations within forty-eight hours when a child got injured on 2/24/25 and received medication attention.

A review of the Employees Who Left Employment in 2024, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

Four employees were missing form 2912 (Pre-employment affidavit).

One child was missing the address for the emergency contact information.

One child was missing the health statement in the file.

It was observed the outdoor area with cat feces. Note: It was corrected at the inspection time when the director cleaned the feces.

Caregivers could not access to children's records upon requested of Licensing. All the files were in the locked office and could be unlocked by the director. Note : corrected. The director came and showed all the files to Licensing. She has one spare key to unlock her office for caregiver can access to children file in case emergency and/or upon request from Licensing.

The center allows parents to pick up/drop off/visit their children from lobby. The parents were unable to come inside the classrooms. The mandatory policy for not entering the facility during the COVID 19 pandemic expired on 04/25/2021.

One driver does not have current two hours of annual transportation safety training.

Based on the information obtained during the investigation, there is sufficient evidence to support that the driver failed to exercise good judgment. The driver did not see the child before she picked the child up for the first time. She did not recognize that they put the wrong child in her vehicle for picking up.

The driver had incorrect dates, time and name on the list of children being transported.

Based on the fire drills log, the last Fire drill documented had been conducted in July 30,2021.

It was noticed that the emergency medical information for one child being transported to the center was not available on the van. NOTE: this was corrected at inspection. The staff made copy of the enrollment form and put it in the binder.
