One staff was not aware of the number of children, in which they were responsible for. The staff informed Licensing that there were 14 school-age children, in which there were only 11 children. The staff acknowledged that they did not account for the children exiting the vehicle.

A caregiver was observed not conducting a physical walkthrough of the transportation vehicle. Caregiver informed Licensing that a child disabled the child safety alarm due to the heat and spacing between the seats.

When I arrived for the follow-up inspection, I observed that the inspection report from the 4-11-22 inspection was not posted at the operation. NOTE: This was corrected during the inspection.

When asked how many children were in her group, a caregiver saif that there were 21 children. There were 24 children in the group.

I observed one caregiver supervising a mixed age group of 24 three yr. olds, four yr. olds and school-age children. Ther specified age of the group was 3 years, and the required child/caregiver ratio for 3 yr. olds is 1 caregiver witn 15 children.

Two out of seven caregivers who have been employed for more than a year did not have annual training in the required topic of SIDS/Shaken Baby.

There was a power strip with three unused outlets not covered in a classroom with four-year-old children. Note- This was corrected during the inspection when the power strip was removed from the classroom.

The director, as well as two caregivers who transport, did not have the required two hours of transportation training. Note- The director, as well as one caregiver, completed the required two hours of transportation training during the inspection.

A two-month-old infant was asleep in a bouncer which is considered a restrictive device. Note- This was corrected during the inspection when the infant was removed from the bouncer and laid in a crib.

15 infant bottles and training cups were not properly labeled with the infant's first name and last initial. Note- This was corrected during the inspection when the bottles and training cups were properly labeled.

The first aid kits in both transportation vehicles were missing a thermometer. The first aid kit in the center was missing tweezers. Note- This was corrected during the inspection when the missing items were added to the first aid kits.

An infant classroom with children 12 months through 17 months of age had reduced ratio during naptime. The classroom had six children with one caregiver which put them over ratio by one child. Note- This was corrected during the ispection when a second caregiver was brought into the classroom.

Two out of seven caregivers who have been employed for more than a year did not have annual training in the required topics of Handling, Storing, and Disposing of Hazardous Materials, Administering Medications, and Preventing and Responding to Emergencies due to Food Allergies.
