The stationery equipment observed was less than 6ft from the gated perimeter.

Two children records evaluated did not have an immunization record on file. One child's immunization was sent via email prior to leaving.

Two children of the primary caregiver who were present during operating hours did not have records available upon request. Based on the information obtained the children are residents of the home.

Two staff present during operating hours did not have notarized affidavits available for review.

One child's record observed did not include their date of birth on the enrollment form. Child's date of birth was updated while licensing staff was present.

There were two pets observed in the home without updated pet vaccinations available for review.

The childcare operation last fire drill practice was documented as January of 2025. There was no documentation for the month of February.

Two children records observed did not include the admission/start date of enrollment. The start/admission date for two children records were updated while Licensing staff was present.

The CPSC form was dated 08/05/2021.

Four children records observed did not include the date of admission. Provider updated start date(s) for children files while Licensing staff was present.

A total of 12 children were observed in care, 12 months- 8 years of age with one caregiver. There were 2 infants, 12 months of age. There were 3 children,18 months of age. There was one 2-year-old in care. There were two, 3 years old. There was three, 4 years old, and one 8-year-old.

Five children under the age of two was observed with screen time activities. Two children, 18 months of age were watching television from a play pen, while the other children were observing from the couch.

A total of 18 out the required 30 annual training hours were observed. Topics that were not observed include: food allergies, guidance and discipline, emergency preparedness, and hazardous materials.

A total of 3 out of the required 24 training hours were observed for one staff/caregiver.

A total of 13 children were observed, 12 months- 4 years of age.

The Licensed childcare home has been operating for 12 months. The primary caregiver has not validated the background check list. There is no current date for the background/employment validation.

The operation had no documentation of their fire drill since 2021. The operation had no documentation related to their emergency practices.

The operation did not have any sign-in and sign-out logs for the children. Note- This was corrected during the inspection when the primary caregiver created a sign-in/sign-out log and signed the children in.

Three out of four children's records evaluated did not have an address listed for their emergency contact. One out of four children's records evaluated did not have emergency medical authorization or documentation showing that the operational policies were received by the parents.

A five-month-old infant who is not yet on table food did not have written feeding instructions on file.

The primary caregiver did not have documentation of training in the required topics of Child Maltreatment and Preventing the Spread of Communicable Diseases.

The assistant caregiver does not have current First Aid/CPR certification.

One out of four children's records evaluated did not have a health statement on file.

The operation has not been documenting their safety drills and carbon monoxide and smoke detector testing. Note- This was corrected during the inspection when the primary caregiver documented the safety drills and testing.

The operational policies did not include policies pertaining to health checks, duration of physical activities, and/or recommended clothing or footing.
