Children's Lighthouse of Spring

Based on interviews and information obtained during the investigation, it was determined that the director used poor judgement by having a child lay on a changing table and be held down in order to administer medication to them.

Based on the information gathered the operation, had a change of director and did not notify Licensing of the change as required.

Based on the information gathered during the investigation, medication was not administered as stated on the label directions. The child was under two years of age and there was no physician authorized dosage documented and signed in the child's file for the medication prior to administration.

The child's medication was not stored in compliance with minimum standards.

While reviewing records it was found that two staff members did not have their education qualifications translated to english.

During the walk through caregiver did not know how many children caregiver was responsible for. Caregiver stated that 15 but there were only 14 children in the classroom. Note: Caregiver had to look at roster to correct herself.

During the walk through one caregiver did not know how many children the caregiver was responsible for. The caregiver counted and stated 14 children when there was only 13 children in care.

In Classroom #8, the inspector noticed a bottle of sanitizer wipes next to the children s drinking cups on the counter, accessible to the children. Note: The director removed wipes from the counter, placing them in the upper cabinet, inaccessible to the children.

The diaper changing mat had a tear exposing the foam, the diaper changing mat looked as if it had been sanitized. Note: Picture was taken.

A caregiver was seen utilizing cleaning supplies in classroom #7, however she neglected to wash her hands afterward. Note- When pointed out caregiver washed hands.

During the walk-through in Classroom # 3 and #6 the caregivers did not know the number of children caring for. Note- Caregivers corrected themselves counting the correct number of children in each classroom.

During the walk-through, it was observed that some of the classrooms were not adequately supervising the children and not maintaining classroom structure.

During the walk-through, a bottle of sunscreen was found under an unlocked cabinet accessible to children in Classroom 6. Note: Corrected at inspection. The director removed sunscreen from Classroom 6.
