One file did not have documentation of that the age and education requirement had been met. 3. One file did not have a notarized affidavit 4. One file did not have documentation of orientation. 5. One file did not have documentation of abuse/neglect training. 6. The orientation documentation in 4 personnel files had not been signed by the trainer.
