At time of inspection, one out of ten child files reviewed. One child was missing current immunization record in their file.

The operation fire inspection expired on 1-07-2025.

At the time of the inspection, one caregiver did not have their required HASA trainings.

At the time of the inspection, director did not have required HASA trainings available for review.

Three caregivers were missing notarized affidavit form 2985.

Director was missing 4 hours of annual training for 2023-2024.

Two children files observed did not have current immunizations on file.

Operation has one pending background check for person that is no longer associated with the operation. During inspection, pending background check for one person was marked inactive by owner.

In one out of the three classrooms observed the classroom was pitch black during nap time.

During walk through one caregiver was observed using their personal cell phone while supervising children.

One caregiver did not know the ages of the children they were responsible for.

Based on external documentation, caregivers did not communicate with one another with how many children they are responsible for.

On 8/5/2024 two caregivers left one child unattending during transition outside. During investigation inspection on 8/12/2024 one caregiver was observed leaving five children unattending in the classroom.

Based on the information obtained during the DFPS investigation, the operation was made aware of possible minimum standards violations and did not make a required report to CCR within two days of the notification.

Emergency Food Allergy Plan for child with birth date 10/7/2021 was not signed by parent or physician.

Medication for child with birth date 10/7/2021 did not have medication in original container.

During inspection, one out of three classrooms observed was too dark to see children.

During snack time one child was not strapped into high chair.

During inspection, two out of the three classrooms observed caregivers did not know the exact ages of each child they were responsible for.

During inspection one caregiver was observed on their personal cell phone while supervising children.

One out of the two vans observed did not have child safety alarm on van.

Toddler playground slide exit was observed at five feet.

Staff hired on 7/19/2023 had CPR training online.

Pre-Kindergarten and School age playground slide exit measured at four feet.
