During inspection, primary caregiver admitted to washing bedding once monthly, when asked how often sheets are washed. Minimum standards requires washing of linens on a weekly basis, unless soiled.

One of the four children's records reviewed did not have an immunization record in file. One of the four records reviewed did not have the emergency's address noted.

Child care home has a pet dog in backyard, which has not had her current Rabies vaccination.

Two of the four children's records reviewed did not have the physician's signed health statement portion filled in on admission form.
