2 children's records were missing health statements for their records

One caregiver was missing five clock hours on their annual training requirement hours.

Director was missing two out thirty clock hours of annual training.

Eight out of nine children files were missing information on health statements, special needs, emergency medical attention, release of children, emergency contact address, and parent s rights.

The operation did not report the number of employees who ceased employment in 2025 by the required date of January 15, 2026.

The grass area on the playground was out grown.

The infant room was observed of having dead cockroaches and debris behind the shelves, milk crate, child size sofa, sitting stools, and between the wall and cabinet. There was a dead cricket in the dining hall under the rolling cart. There were dead cockroaches and crickets on the floor with collected debris. There was a spider with its spider web on the windowsill, dust and dead cockroach.

The toddler classroom has a diaper changing station and no hand wash sink.

The infant caregiver walked out of the classroom to open the front door, leaving two infants alone while they were sleeping in their cribs.

The operation does not have liability insurance and they had no proof of notifying the parents that the opeation does not currently have liability insurance. Note. The parent handbook has a date stamp of 9/19/2025 of being modified.

There was a basket filled with monster trucks designated for children ages 3 years and older. The youngest child in the classroom was two years old.

The infant room was observed of having dead cockroaches on the floor and window seal, spider webs, used Q-tip, and debris on long the side of the wall and also on one crib mattress. There were other classrooms also with dead cockroaches on the floor.

The infant area has two rooms adjacent; one side with cribs and the other with toys, infant furniture and diaper changing station. The room with the cribs was dark during naptime and had two infants sleeping in their cribs. Note. This was corrected when the caregiver opened the curtain to allow light in the room.

The toddler classroom is currently under renovations and has been moved to another room where there is no hand wash sink. There is a diaper changing station in the classroom in which caregiver stated they use hand sanitizer after diapering. Note. A portable hand wash sink was added to the classroom.

The toddler classroom for 18-23 month old children had magnetic Bingo chips designated for ages 3 and up. Note. Although the classroom has a combine group of children ages 2 -4 years old, the classroom is still considered and approved for the toddler children. Therefore, the toys and equipment must meet age appropriate guidelines.

At the time of the walk through inspection, a group of children in the infant classroom were observed having screen time. One child was two years old.

The operation recently had their chain link fence replaced, leaving several areas with at least a 10-inch gap from ground to fence.

There was a hole on the wall exposing electrical wires to an outlet. This poses a risk to any infant, but especially the one who is laid down on a crib within hands reach.

The trash can in classroom #5 had the lid removed and found inside the can. There was a trash bag filled with garbage that was not taken out on the last operating business day. Classroom #4 was observed with several different sizes of dead cockroaches on the table, tile floor and rugs. Note. This was corrected at inspection when the trash bag was removed and replaced with a new bag and lid placed over the can.

The restrooms located in the hallway did not have soap in the dispensers. Note. This was corrected at inspection when the soap dispenser was refilled.

The operation does not have liability insurance and there was no posting or any other notification staying they have no liability insurance.

At the time of inspection, one infant did not have a current feeding log for the month of June. The infant went from drinking breast milk to formula.

A caregiver disregarded the feeding log a parent had recently updated for the month of April and served an 8-month-old infant cranberry juice. The juice was not listed on the feeding log.

An infant's feeding instruction was not followed when a staff served an infant cranberry juice.

There were no feeding logs available for review in the infant classroom for infants not yet eating table food.

The files of three out of four children were reviewed and had missing vaccinations according to their age.

Two infants feeding instructions were filled with information on the type of formula, number of bottles and ounces to service, but did not have the date and parent signature.

The transportation van did not have the child safety alarm in operating conditions.

The staff files were reviewed and found incomplete. Staff did not have the required annual training hours.

Six out of seven staff files reviewed did not have annual training hours. One of those staff files was the infant caregiver who did not have SID's, Shaken Baby, Brain Development and Child Maltreat training.

The gas inspection has not be conducted since November 2021.

School age children were observed walking from the playground back inside the building to use the bathroom or allow parents into the building while the caregiver stood between the door of the playground and the building.

The second director has not been submitted as a controlling person. She was listed as a director in November 2023.

An infant caregiver had children eating snack watching Cocomelon on a tablet.

There was a four month old infant sleeping on a restrictive device inside the crib with a blue fleece blanket. Note: Corrected on 5/22/2023 during inspection, when caregiver removed the infant from the restrictive device and out of the crib.

The fire inspection was delinquent as of 11/10/22.

While reviewing children's records, 1 of 6 children's files did not have a medical authorization section filled out and signed by parent.

Snacks were observed being served on bare table for 2 infants during snack time.

During inspection it was observed that a 3 month old was asleeping in the crib with a blanket inside the crib.

The center did not have a current gas inspection. The last gas inspection was conducted 6/14/2019.

During inspection it was observed that a 3 month old infant who is not able to turn on her own was asleep in the crib on her stomach.
