During the course of the investigation, it was determined that the children were left unsupervised in a classroom for 1 minute 15 seconds.

Based on the information gathered, the group transitioned back into the building and in the process a child walked into an empty classroom, while the rest group transitioned back into their classroom, leaving the child unsupervised.

During the walkthrough, a used glove was observed on a changing table mat. NOTE: This was corrected at inspection when the glove was removed and the area was sanitized.

During the walkthrough, diaper cream was observed being stored in the restroom and was accessible to children. NOTE: This was corrected at inspection when the diaper creams were removed and stored out of the reach of children.

The classroom cots were observed to be numbered but the classrooms did not have corresponding cot charts or name lists. NOTE: This was corrected at inspection when cot lists were made for each classroom.

The operation failed to notify parents of three children who received minor bite and scratch injuries while in care at the time of pick up.

Classrooms did not have planned activities scheduled or being followed.

The Toddler room had a loose cabinet knob and laminate that was peeling off of the counter. The infant room had a missing ceiling tile. There was a trash bag with old food by the fence on the open field side of the playground. A fence gate on the back playground next to the pool area has a gap over 5 inches from the ground to the bottom of the gate. A bathroom drain cover was not attached in the Prek classroom. An electrical control box for the door key pad off of the exit door to the back playground could be removed, exposing wires.

Two children with medication on site did not have the parent signature authorizing the medication be given by the operation.

Two children with medication on site did not have the original prescription accompanying the medication. NOTE: One child's parent brought the prescription box during the inspection.

One child with medication on site had expired 5/25.

Two children with allergy plans on file did not contain a doctor signature.

A child was fed a Nutrigrain bar that contained allergens listed on the child's food allergy plan.

A caregiver was present and in direct supervision of children in care without a background check being entered or an eligibility letter on file. NOTE: Caregiver was sent home during the inspeciton and a background check was entered.

A child was observed sleeping with a sippy cup in their mouth. NOTE: The caregiver was asked to remove the cup during the inspection.

RECITE: A seatbelt on Vehicle #3 has not replaced the seat belt that was inoperable and covered it with red duct tape. The Facility Manager was present during the inspection and scheduled a mobile tech to come out on 5/14.

A seatbelt on Vehicle #3 had a frayed seatbelt prohibiting it from being used.

There was peeling paint on the wall and window sill in the Toddler room.

The large playground climber measures 9 ft in height at the highest platform. NOTE: A waiver request was provided during the inspection.

An outdoor outlet on the Toddler playground did not have a cover. The fence around the ac units on the back playground was curling allowing children to climb under or for clothing to catch on the fence. A utility knife was found open and sitting on top of a sensory table. NOTE: The knife was removed from the playground during the inspection.
