Three infants were sitting in highchairs being fed by caregivers and straps were not fastened. Note: This was corrected at inspection when the caregivers fastened the straps.

Based on information gathered during this investigation, a caregiver used inappropriate punishment by restraining a child for an extended period of time.

Two children were able to exit the classroom and were unsupervised for approximately less than five minutes.

A caregiver who was inactive was present in a room. This was corrected at inspection when the caregiver was removed from the classroom.

At the time of the walkthrough into the young preschool A classroom there was a 17-month-old with a three-year-old making that a 19-month difference in age. NOTE: This was corrected when the 17-month-old was removed.

At the time of the walkthrough the PreK teacher said there were 16 children when we verified there were 14 children.

Video footage reviewed during this investigation showed a caregiver grabbing and yanking three children aggressively by their arms to the floor to force them to sit. She was additionally seen grabbing and dragging a child by the legs towards herself in order to hold the child still by holding her between her feet and lower legs.

Video evidence collected during this investigation showed a caregiver repeatedly mistreat an individual child in care while interacting more positively with other children. The child was observed upset and seeking comfort from the caregiver who repeatedly pushed her away with her hands and feet.

Video footage reviewed showed a caregiver demonstrating poor judgement and self-control while caring for children at the facility. She was seen pulling, pushing, and dragging children who were not behaving as she desired during the 15-minute video with both her hands and feet. She was also observed restraining a child who was laying on the floor by holding her torso between the caregiver?s feet.

Three of the ten children were missing healthcare statement. Three of ten children were missing updated immunization records.

Two of the ten children were missing special care information. One of ten children were missing release information. One of ten children was missing emergency address.

Two of the asthma medications were observed expired 11/2024 and 4/2025. NOTE: Corrected at inspection when one medication was disposed.

Based on information gathered and interviews conducted, inadequate supervision was determined to have occurred when a child was seen standing outside the door unsupervised when the child was quickly found after the class entered the building from the playground.

During the investigation, it was noted that the administration failed to promptly inform the parent regarding the alleged inappropriate discipline used with child.

The operation did not promptly notify CCR about the alleged inappropriate disciplinary practices used by a caregiver.

At time of inspection, director did not have a current drivers license on file.

At time of inspection, a childs file did not have food allergy action plan.

At time of inspection, a preschool class was observed with gardening fertilizers in a cabinet at childs reach. Corrected at inspection.

A staff member's who transports children was found to have an expired 2022Texas Drivers license.
