Two caregivers were observed utilizing their cell phones while their classes were on the playground.

During the course of the investigation, it was determined that a caregiver used inappropriate discipline by striking a child's hand as redirection.

During the walkthrough, an infant was observed sleeping with a bib on. Note: The bib was removed immediately

At the time of inspection, one out of the six children's files did not have a Physician's Health Statement for review.

During the walkthrough, a caregiver stated there were nineteen children in care when there were twenty. Note: this was corrected when the children were recounted

The permit holder failed to notify CCR that a written notice from a governmental entity indicating that the holder's corporate privileges are forfeited and/or in danger of forfeiture.

The center's director left the operation to run errands, leaving a caregiver alone with 23 children, including an infant.

During the inspection, multiple children were observed drinking out of cups and eating foods while walking around.

During the inspection, there were multiple plugs missing covers in the classrooms. NOTE: this was corrected at inspection when outlet covers were placed on the plugs.

During the walkthrough, multiple children were observed climbing on tables, bookshelves, blocks, and playing in the restroom.

One caregiver was supervising 23 children. The specified age for the group was three years old meaning the caregiver could only have 15 at one time.

At the time of inspection, two out of the six children's files reviewed did not have current immunization records for review.

One out of six children did not have the health care information listed in their file.

Six out of the six staff member files showed that they had only the online portion of the CPR certification.

One of the caregivers did not have first aid training at the time of inspection.

Caregiver had 23 children in care, ranging from 17 months to 10 years of age. The difference in age between the youngest and oldest was greater than 18 months.

One out of the six children's files reviewed did not have complete information for authorized pick up persons.

At the time of the inspection, a caregiver was observed using a cell phone to print labels. Another caregiver was sending text messages about children's arrivals throughout the morning, instead of signing children in.

Two out of the six children's files reviewed did not have complete emergency contact information.

During the inspection, the sheets on the infant cribs were too small for the mattresses, causing the mattresses to fold over on themselves.

Caregivers had 18 children in care, the youngest being 16 months old and the oldest child in the group is an 8 year old, therefore the youngest/oldest children present are more than 18 months apart.

During the walkthrough, there were multiple children sleeping with their milk cups.

During inspection it was observed that an infant was walking around drinking his bottle.

RECITE; Caregiver had 17 children in care, and one child is 15 month old and the oldest child in the group is an 8 year old, therefore the youngest/oldest children present are more than 18 months apart.

Director did not have an admission form for one child in care.

During inspection caregiver stated that she had 15 children in care, and after counting it was determined caregiver had 16 children in care.

One classroom did not have a smoke detector.

Recite; Caregiver in the duckie classroom had 17 children in care. Based on the specified age group caregiver can only have 15 children in her group. Caregiver in the lambs room walked out of the infant room, leaving caregiver out of ratio with 7 infants.

Four infants did not have updated infant feeding instructions.

The lambs classroom was observed out of ratio, caregiver had 5 children ages 3 month, 5 month, 8 month, 8 month and 12 month. Based on the specified age group caregiver can only have 4 infants in her group. Caregiver in the duckie classroom had 19 children in care. Based on the specified age group caregiver can only have 11 children in her group. Note: Two caregiver's arrived to assist with ratios.

The infant cribs were not labeled with child's name. Note: Corrected at inspection when caregiver labeled the cribs.

The fire extinguisher has not been serviced, as required.

Dust can be seen accumilated by an air vent. Empty spider webs were observed in a playground structure.

A blanket was observed in the crib with a 2 month old.

Garbage was observed accessible to the children in care as a trash can in the children's restroom was not covered. Note: This was corrected at inspection as the trashcan was covered.

Good judgement was not exersiced when a two year old child was strapped to a high chair placed on an inadequate small chair.

Wooden boards observed broken in the playscape, The slide on the same play structure was observed cracked resulting in a gap. Nails were observed protruding from the playscape.

Children observed did not wash their hands before breakfast. Note: This was corrected at inspection when other children were directed to wash hands before eating.

An infant was observed sitting in a baby walker. Note: This was CORRECTED AT INSPECTION when the walker was removed from the infant classroom.

Thick layers of dust built up were observed on vents through out the center.

A caregiver was observed caring for children without an eligible background check. A 16 year old was observed at the operation without an eligible background check.

Infant's individual storage space must be labeled either with the child's name, photo or symbol.

A blanket was observed in the crib with a 10 month old.

The operation had an outstanding balance of $4 for their background checks and it was paid during insepction.

The parent contact authorization forms were not observed in the vehicle for four of the children being transported by the operation.

Two five years old were observed transported by the operation staff without a booster seat.

The emergency medical transport and treatment authorization forms were not observed in the vehicle for four of the children being transported by the operation.

Based on the evidence gathered during the investigation, a three year old was transported without the proper front facing child safety seat.

A cord to the ac mounted in the window located in the three to five year old classroom was observed accessible to the children in care.

A hole in the ceiling and mold were observed in a closet located in the three to five year old classroom. Blinds in the three to five year old classroom were observed dirty.
