Three outside plastic toys (two lawnmowers and an infant activity gym) were observed to be broken with sharp edges and cracks. NOTE: Compliance has been met as the items were removed.

One of the six children's files was missing the immunization records.

At the time, the playgroound surfacing appeared to be compact with no 6 inches of mulch in the use zone areas of the slide and exit areas.

Three of the six caregivers were missing the affidavit form 2985.

During naptime in the toddler 2 classroom, there was a total of eight children. There was a 17-month-old included in that total, which is considered an infant. Reduction for naptime is only allowed for 18 months and older, which does not include doing the specified age. NOTE: This was corrected during inspection when another teacher came into the classroom

The first aid kit had two expired bottles of peroxide. NOTE: This was corrected at inspection as they were removed.

Two of six children files evaluated did not have current immunization records available for review.

Cots are not labeled or assigned to children.

Four caregivers does not have the required training in the following topics:Administering Medication, Preventing and responding to emergencies due to food or an allergic reaction; and Handling, storing, and disposing of hazardous materials including compliance with ?746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?)

Two of the six children records didn't have a current immunization as DTap were missing.

Two of the six staff were missing the following training for 2022-2023 Poison Prevention Medication and Food allergy.

According to the last fire inspection it was last conducted on 10.25.22. NOTE: Fire inspection has been set up for 11.2.23.

The last fire drill and extinguishers were conducted on 7.24.23.

No documentation for the fire extinguishers, smoke detector or carbon monoxide was available at inspection. NOTE: This was corrected at the time of inspection

Six of the six files did not have photo identification/ID's available in the files.

Three of the six children files did not have updated immunization records.

Diapers and wipes located under the changing table in the infant and shared restroom were easily accessible to the children.

Two of the six employees had expired CPR/First Aid trainings.

During the walkthrough into the mobile infant classroom, the children were observed being seated at the table without washing hand before lunch was served. Based on the information gathered and interviews conducted the infant caregiver was unaware of having to wash the infant's hands before feeding infants their baby bottles. NOTE: This was corrected at inspection.

Based on the information gathered and interviews conducted at the time of the inspection the caregiver and director determined the television was viewed by the children during breakfast.

During the walkthrough on the playground we observed trash can with no cover and full of trash that is accessible to children and could cause a safety hazard. We also observed a broken asphalt on the playground that can cause a tripping hazard for toddlers. Trash was accomulate on the ground outside of the fence.

The emergency preparedness plan did not specify procedures for evacuating infants under 24 months of age, who have limited mobility, or who otherwise may need assistance in an emergency, such as children who have mental, visual, or hearing. -Continue care and -Reunifying

The fire inspection was last conducted on 9/10/20.

The director was missing the following training; Shaken Baby syndrome, safe Sleep, and Brain development. -Emergency preparedness -Understanding building premises and -Handling and storing hazardous material

Three of the six children files did not have the Name and address if the Healthcare Profession.

Four of the six caregivers did not have the following training available for review; -SIDS, Sudden infant, and brain development. -Emergency Preparedness plan -Food allergy -Understanding building premises and -Handling and storing hazardous material
