The transportation binder did not have emergency medical transport and treatment authorization forms for each child being transported.

There were outlets observed uncovered in multiple classrooms. There were nails protruding from multiple points on the playgrounds. There was a rock wall that was too high for the children who utilized it. Note: The outlets were covered during the inspection.

During the playground inspection, there was a slide that did not have a use zone of six-foot use zone in front of it and a four-foot use zone around it. Note: This was corrected at inspection when the slide was removed.

There was a free hanging rope swing observed on the playground.

On the top of the PreK classroom sink in the classroom accessible to children was a bottle of hand sanitizer, hand scrapper, three step chemicals bottles and a can of bathroom scrubbing bubbles. NOTE: This was corrected when the items were removed into a high cabinet.

One of the six children was missing an updated immunization record.

A yellow wood entrance door into the outdoor stage area was observed off the hinge. In addition to a maroon wood sliding door outside that leads into an area with table.

The school age teacher was unaware of the number of children in care as those children were transitioned by another teacher to that teacher.

In the toddler classroom the teacher was observed serving cookies to the children on the bare table. NOTE: This was corrected when napkins were being used.

The caregiver with the afterschool children stated there were 18 children before verifying there were 19 children in care.

One of the ten children files was missing emergency contact address. In addition to if the child has a food allergy.

A 14-year-old was observed directly working with the children in the toddler classroom. NOTE: This was corrected when the child was removed.

Thirty hours of required training for the second director was unavailable for review for 2022-2023.

Based on the interview conducted it is determined by the caregiver that at times while the children are sleeping the caregiver is reading a chapter book.

During naptime in the mobile infant classroom one caregiver was supervising nine children with the youngest being 12 months and the oldest being 18 months old as they were napping. NOTE: This was corrected when another caregiver was called into the classroom.

Two caregivers didn't have sudden infant death syndrome (SIDS) annual training for 2022-20223.

An inhaler for a current child was verified to be expired on 4/2023. NOTE: This was communicated to the parent and parent requested it be disposed.

One of the six staff was missing 7 hours of annual training for 2021-2022.
