This was partially corrected when the infant caregiver moved the 9 month old to the infant room but the 5 year old was remained unsupervised in the classroom. A 3 year old then left the restroom and entered the classroom unattended. The 3 year old and 5 year old were observed unsupervised in the classroom. This was corrected when the director and the rest of the group returned to the classroom where the 5 and 3 year old were.

Based on the evidence gathered, one of the directors does not maintain attendance records or time sheets for themselves and could not provide this documentation upon request.

Based on the evidence gathered, a group of children with a specified age of 2 years old was out of ratio by one child for at least 10 minutes on 03/18/2025. One caregiver had 12 children in their group.

A television was observed not mounted during inspection.

The cleaning sprays located in the cabinet in the restroom used by children were accessible as the child proof lock was unlocked. Note: This was corrected at inspection as the cabinet was locked with the child proof lock.

A nine month infant was observed sleeping in a restrictive device. Note: This was corrected at inspection when the infant woke up when being removed from the restrictive device.

Two out the four files reviewed did not have current immunization records.

One out of the three staff records reviewed did not have current First Aid and CPR training.

A staff member used too much bleach when mopping the floor, causing a child's pants to get bleach stains when child crawled on the floor.

Based on information gathered during the course of the investigation, a caregiver was on her phone while supervising the children in care.

3 infant bottles were not labeled.

An 11 month old non-walking infant was observed sleeping on a mat.

2 infant feeding schedules observed had not been signed by the parent.

The infant changing mat was observed to be torn. Note: This was CORRECTED AT INSPECTION when the old changing mat was removed.

A background check fee has not been paid.

A child under the age of two years old was observed watching television. Note: This was corrected at inspection when the child was moved.

Caregivers did not wash their hands before feeding children and after removing gloves.

Meals were not included in the operational policies.

The infant safe sleep policy was not included in the operational policies.
