During an investigation, it was determined that a caregiver failed to ensure an exterior door to an empty classroom was securely locked behind her which resulted in children entering an empty classroom with no supervision. Caregivers outdoors failed to provide adequate supervision to become aware that children had entered the building unsupervised.

One of ten records did not have an up to date immunization record.

During a DFPS investigation, it was discovered that the operation failed to inactivate an employee that has not been associated/employed with the operation for over seven days.

During this investigation, it was determined that caregivers left a child unsupervised while the classroom of children went outdoors to the playground.

Seven of eleven personnel files did not have their annual training records available for review. Seven of eleven records did not have an employment start date.

A new employee that began employment as a caregiver in a classroom did not have First Aid certification.

The emergency practices log for last school year 2022-2023 had documentation for only two drills conducted all year.

During this investigation, it was determined that a caregiver left a child unsupervised, allowing the child to wander to another classroom.

During the investigation, it was discovered that CCR was not notified within the required timeframe of 48 hours. Operation was made aware that child would be taken for medical treatment.

The director is missing 24.5 annual training hours from 2021 to include child development, discipline and guidance, curriculum, teacher-child interactions, serving children with special needs, abuse and neglect, emergency preparedness, preventing the spread of communicable diseases, administering medication, preventing and responding to food or allergic reactions, physical safety and hazards, handling hazardous materials, and three hours management.

A caregiver grabbed a child by the forearm when the child refused to nap, resulting in two fingernail abrasions that broke the skin.

Three of four staff files audited did not have current CPR training.

Four of four staff files reviewed did not have training for handling hazardous materials or administering medications.

Three of four staff files audited did not have current first aid training.

One preshool classroom had missing electrical safety outlets. This was corrected at inspection when director added to the safety plugs to the electrical outlet.

In one of the preschool classroom an electrical fan was unmounted and in easy access to the children. This was corrected at inspection when caregiver removed the fan and placed it in the closet.
