An employee with a provisional background check status was placed in charge of the toddler class by herself.

Five of the nine staff files did not have their required training hours completed.

The operation did not submit the required number of employees lost during the year 2023, by the due date of 1/15/24.

During inspection, there was a tablet stood up on counter showing a screen time activity for infants to watch. Two infants were in play yards in front of counter and were actively viewing the program. NOTE: This was corrected during inspection as the caregiver turned off the tablet.

It was observed that a monthly fire drill was not documented as conducted for the months of April, July, and August.

An employee who has had unsupervised access to children at the operation was certified only for BLS not pediatric First Aid.
