All four infant bottles in the Infant 0-6 month old classroom had no labels on the bottles with their names.

During the investigation, it was noted school-aged children were napping on the class floor.

Throughout the investigation, it was observed that the operational policy lacks provisions for the processes of suspension and expulsion.

During the course of the investigation, it was noted operation staff failed to provide adequate supervision due to daily incidents where children are fighting among themselves while in care.

During the inspection, it was determined that an individual present at the operation does not have a valid background check. Note: This was corrected when background check was re-submitted to change Role Type during inspection.

During the course of this investigation, it was found that the staff member who accompanied the child in the ambulance to the hospital had not completed a valid background check for this operation.

One caregiver in classroom D-12 was observed working in the classrooms supervising children without receiving a background check and letter of eligibility. NOTE: This was corrected at inspection when the caregiver left the operation and a background check was submitted.

At time of inspection multiple caregivers were unable to provide the ages of the children in their class.

At time of inspection two toddler caregivers were unable to provide the ages of the children in their class.

During the inspection, it was observed that infants seated in restrictive devices were not properly secured with straps. This was corrected at inspection.

Napping children were observed placed in front of exits in multiple classrooms. This was corrected when the director moved each child away from the exits.

In toddler room, the children were served crackers without a plate or napkin. This was corrected when it was addressed with the caregiver and caregiver provided plastic plates for the children to eat their snack on.

A caregiver did not wash an infant's hands after changing their diaper. This was corrected when the caregiver was reminded about handwashing procedures and the caregiver washed the infant's hands.

Cleaning solutions and chemicals were observed under the sinks in three children's restrooms without child safety locks.

A vehicle that is used to transport children is not equipped with an electronic child safety alarm.

A piece of outdoor play equipment was observed in front of a slide on the playground. The equipment overlapped the slide's required use zone.

An infant classroom with a diaper-changing area does not have a handwashing sink.

The following personnel records were not available for review at time of inspection: proof of education, notarized Affidavit for Applicants for Employment form, proof of background check eligibility letter, copy of photo ID, and signed statement employees recieved copies of operational policies and employee policies.

Children were observed using a restroom in a room filled with stacks of boxes and other storage items.

Crackers were served on a bare table for children to eat from.

Based off information gathered, a parent wasn't notified at pick up the same day their child was bitten by another child. The parent was notified two days later.

Based off information gathered, a child was bitten by another child when the classroom was out of ratio by one child. There were 6 infants present with a specified age group of 15 months with only one caregiver.

Five of ten children's files did not contain up to date immunization records.

Three of twenty two caregivers did not have a current CPR/First Aid certificate on file.

During inspection it was observed that one caregiver had 8 children in care ages 8 month, 9 month, 9 month, 9 month, 10 month, 10 month, 12 month, and 18 month. Based on the ages of the children caregiver is out of ratio.

Caregiver's used poor judgment when they failed to touch the playground equipment that is unshaded, ensuring the playground equipment was cool enough for children to play. This resulted in a child receiving a burn on abdomen area.

Two fire extinguishers in the center vans have not been serviced, as required.

The operation did not have a current fire inspection. The last fire inspection was conducted on 8/3/2020.

Based on information gathered, parent was not informed that child was left unsupervised for 1 minute on the playground.

Based on information gathered, the center did not make a report to CCR, that a 2 year old child was left unsupervised for 1 minute on the playground.

Based on information gathered during the course of the investigation, it was determined that a 2 year old child was left unsupervised for 1 minute on the playground.
