A current gas inpsection was unavailable for review. The expiration date was 7/25/23. During the inspection the assistant director made an appointment for 6/30/26 with Big City Plumbing.

School age children were observed to be freely watching Youtube videos. The video that was being watched appeared to be Spongebob with music dubbed over the scene. This was corrected during the inspection when the tv was turned off.

It was determined that caregivers did not demonstrate good judgment and self-control when conversations between staff members, which were had in front of children, resulted in staff using curse words.

It was determined in an investigation that a caregiver was smoking on the operation property.

Staff were missing trainings in emergency preparedness, child abuse, preventing communicable diseases, administering medication, responding to emergencies, building safety, and handling hazardous materials.

The director was missing training in SIDS/shaken baby/early brain development, emergency preparedness, preventing communicable diseases, administering medication, responding to emergencies, building safety, and handling hazardous materials.

The director did not immediately call EMS when an infant received a head injury severe enough to cause bleeding from the ear.

Based on the DFPS investigation, a video of the incident was observed. On the video a child was observed coming out of the bathroom and not washing their hands before rejoining the class.

On the video a caregiver repeatedly put their hand on the chest of an 18-month-old child so the child would remain inactive for approximately 26 minutes. The child was required to remain seated and not allowed to get up or interact with other children.

Three of ten children's folders did not have an address listed for the emergency contact.

A child with known food allergies did not have an action plan on file.

A first aid kit was missing tape and a thermometer.

One of ten children's files did not have a current immunization record.

Video shows a caregiver failing to wash the hands of a child they diapered.

Video shows that the safety straps were not used for a child who was placed in a highchair.

A caregiver did not demonstrate good judgment when they failed to use the required safety straps to buckle an infant into a highchair.

A caregiver was observed failing to wash their hands after diapering a child.

Video shows a caregiver not appropriately supervising an infant in a highchair when the caregiver is observed sitting down in a blind spot and not keeping the child in view.

Bucket swings in the toddler playground were found not having the 6 feet of use zone on the side of the adjacent swing and the fence. *Swing was removed immediately.

Nine bottles, in the Lambs Classroom were not labeled with the child's first name and last initial. *Note all bottles were labeled at the time of the inspection.

Based on interviews the operation was using threats for discipling purposes. Children were being told that if they did not behave they would be taken to a specific caregiver.

The two playgrounds (12-18 months and 2 year old) in the operation were noted to have gravel covering concrete.

The operation did not provide an incident report for an injury that required medical attention.

Three out of three transportation vehicles fire extinguishers were not securely fastened. It was noted that the fire extinguishers were loose in the vehicles floors.
