The operations fence surrounding the pool has one picket that has become detached at the bottom joint and needs repair. Note: This was repaired during the inspection

Two caregivers in the school age room did not know how many children were in the classroom when asked during an inspection.

The changing table in room 12 is damaged with multiple holes in the plastic covering. Note: This was corrected when a new undamaged changing pas was replaced in the classroom.

One trash can in room 1 was seen with garbage accessible to children. Note: This was corrected during the inspection when the trash can was replaced with a can that has a lid.

Five of five infants seated in a multi-seat toddler table did not have their safety straps fastened.

A caregiver did not use good judgement when she failed to read the manufacturer's instructions on the Magic Eraser before using it to remove marker from a child's face. The cleaner contains a chemical agent that resulted in chemical burns to the childs skin.

During a DFPS investigation, it was determined that physical abuse occurred when a caregiver did not read the instructions for a chemical agent (Magic Eraser) and proceeded to remove marker from the child's face which resulted in chemical burns.

During an investigation, it was determined that an employee did not use good judgement when taking a child in care up to the second level. A caregiver did not use good judgement in allowing the child to be taken out of class at the time.

During a DFPS investigation, it was determined that the operation failed to report concerns expressed by a parent that require investigation to Child Care Regulation as required.

During an investigation it was determined that the director did not meet responsiblilities when a parent informed her of concerns they had involving an incident with their child and an employee.

During an investigation it was discovered that an office employee took a child up to the second level while in care. There is no written approval that care of children is allowed on the second level.

There were two trash cans that were observed to have no lids on them, that had soiled/dirty diapers thrown in them. Note: This was corrected at inspection where there were lids that were placed on both trash cans.

There was an infant feeding schedule that had not been updated since 8/18/2022. Note: This was corrected at inspection when the mother of the child completed and updated the form.

After a child was diaper changed the child was observed to be placed back in their chair without getting their hands washed. Note: After the caregiver was reminded to wash the child's hands, the caregiver quickly went to the child and took them to go get their hand's washed.

It was determined that a caregiver forcibly shoved a child's neck down to show the child the food that fell on the floor. Additionally, it was also determined that the same caregiver had force fed a child.

It was determined that the caregiver in the classroom was yelling at the children.

During the course of an investigation conducted by DFPS, It was determined that a caregiver physically abused a child in care by forcing their head down to look at a cupcake that the child had dropped on the floor.

It was determined that a caregiver forcibly corrected the way a child was sitting in their chair.
