At time of inspection, a toddler caregiver stated they had 10 children when they had 9.

During the inspection the nap time mats were numbered, but a chart was not available to know which child was on which mat.

The fire extinguisher in the kitchen was last serviced on 1/2024.

The annual fire inspection was expired. The inspection occured on 11/19/2024.

The children in the toddler room ranged in ages from 12 months to 3 years old. Only 18 months difference in age is allowed.

Two cribs in the infant room contained napping mats, blankets, sheets and soft toys. These were removed at the inspection.-

A large tree branch was found on the playground.

During the course of the investigation, it was determined a caregiver allowed the children to watch tv while waiting for the rest of the class to arrive.

The caregiver in the naptime room didn't know the number of children they were responsible for.

Three children's files reviewed did not have health statements.

A group of nine children, who were having their AM snack, were engaged in screen time during their meal.

A spring play structure (green plane) does not have the six foot use zone around the equipment.

One staff member did not have a notarized affidavit available for review.

A group of nine children, ranging from toddler - prek, was left unsupervised when the caregiver left to grab her personal belongings.

In a room with a specified age group of 2 years old, there was 12 children with one caregiver.

Recite: The swing set's use zone overlaps the uze zone of the composite structure and has a table within the use zone.

Two children were placed in low sitting feeding chairs and their safety straps were not being utilized.

After diapering an infant, a caregiver was observed to not wash her hands after the diapering was completed.

A caregiver was observed taking a phone call from her personal device while being counted in ratio.

An infant (who is able to sit unassisted) was not taken to the sink to wash their hands.

After completing a diaper change, the changing surface was not sanitized before utilzing the surface to change another child.

The operation did not have proof of insurance during an inspection and failed to provide it by the previously discussed date of close of business, March 5th.

The operation did not notify CCR staff before changing the infant spaces.

In a room with children ages 10 months-24 months the television was being utilized to play a movie.

The swing set's use zone has other pieces of equipment within it.

One of six children's files did not have a signed medical authorization included in the file. One of six children's files did not have an address for the emergency contact listed.

Two of six children's files did not have an up-to-date immunization record on file. Two of six children's records did not have a signed physicians statement in the file.
