According to the Texas Comptroller website, the business entity's Franchise Tax Account Status is Inactive/Forfeited. The operation is currently providing care to children with an inactive status.

It was determined that three caregiver's did not have first aid and CPR training.

It is determined that one caregiver does not have a valid High School Diploma/GED in file.

The director is not ensuring the center is operating in compliance with MS when the director did not verify multiple caregivers education credentials. The director also did not ensure that First-Aid/CPR cards provided to her upon hire were legitimate

Based on information gathered, it was determined that fire drills are conducted every two months.

During the walkthrough, an infant was observed sleeping with a bib on. Note: The bib was removed immediately

The driver failed to provide a child with the required booster seat during transportation.

A trash can observed on the playground containing garbage did not have a lid. Note: This was corrected at inspection when a lid was placed on the trash can.

Two mattresses were observed stacked on top of each other in a crib in the infant classroom. Note: This was corrected at inspection when the bottom mattress was removed leaving one in the crib.

Crib sheets were observed to be loose fitting.

Cleaning supplies were observed in the cabinet underneath the sink in the children's restroom with no child proof lock making them accessible to the children in care. Note: This was corrected at inspection when the cleaning products were removed out of children's reach.

A caregiver in the Toddler room did not know the ages of the children in their care.

Bandages and a thermometer were missing from the van first aid kit. This was CORRECTED AT INSPECTION.

The weed prevention mesh under the pea gravel was observed to be pulled up. This was CORRECTED AT INSPECTION when the mesh was cut.

Children were observed eating snack while watching tv.

The weed prevention mesh under the pea gravel was observed to be pulled up creating a tripping hazard. Trash was observed on the playground. A gate leading to the playground was observed to be broken.

Caregivers in the infant room did not know the ages of the children in their care.

A person without a cleared background check was observed caring for children. Note: This person was sent home.

During the course of the investigation, it was determined that a caregiver was observed to be asleep when caring for a group of children. Note: The caregiver is no longer present at the operation.

Parent's Rights were not added to the operational policies. Note: This was corrected at inspection when the operational policies were added.

The lighting in the classroom was not adequate. Note: This was corrected at inspection as the Director the light was turned on.

Three cribs were not labeled with child's name. Note: Corrected at inspection when director labeled the cribs.

The caregivers did not provide the level of supervision necessary to ensure each child?s safety and well-being, including physical proximity and auditory or visual awareness of each child?s on-going activity when they were outside playing in the playground.

Caregivers and directors are not recording accidents or incidents that children are involved in on the Licensing Incident/Illness Report Form 7239 or another form that contains at least the same information.

Caregivers are not completing the incident reporting forms.

The operation failed to notify CCR as soon after the incident as possible, but no later than two days after.

The operation did not provide/share and incident report with parents of children who escaped the playground through a side gate.

The operation failed to notify parents of the situation their child was in when the children escaped through the side playground gate.

The operation did not have the current employees list, menu and daily activity schedule posted. Note: This was corrected at inspection when the required documents were posted.

Swings were observed attached to a composite structure.

There was a prohibited play structure observed on the playground.

Trapeze bars were observed on long chains in the playground during inspection.

No loose fill surfacing material was installed under the slide.
