During investigation it was found, a child was able to open the door and go onto the playground and was unsupervised for less than 1 minute.

Two of ten immunization records were not current.

A two month old was combined in a group with a 22 month old. This was corrected at inspection when the 22 month old was removed from the class and replaced with a 14 month old.

Exposed piping was observed in the school age back play area. These exposed piping are a tripping hazard and need repairing.
